The Australian Department of Health ('Department') is collecting personal and sensitive information about you to enable a response to your enquiry regarding the Breast or Bowel Screening Programs.

If you do not provide sufficient detail with respect to your question, the Department may not be able to properly respond to your request.

The Department may disclose the details of your question to state and territory government health departments, the Department of Human Services, and the contracted pathology service providers for the National Bowel Cancer Screening Program.

  • how you may access the personal and sensitive information the Department holds about you and how you can seek correction of it; and
  • how you may complain about a breach of the Australian Privacy Principles (‘APP’) or a registered APP Code that binds the Department, and how the Department will deal with such a complaint.
The Department will not disclose your personal and sensitive information to overseas recipients.
